Please arrive 15 minutes prior to your scheduled appointment to complete any necessary paperwork.

Download and fill out your forms at home to save time at check-in.

We work with a wide range of providers to ensure quality care is accessible to everyone.

We accept all major credit cards, HSA/FSA accounts, and offer flexible payment plans for uninsured patients.

Please provide at least 24 hours notice for cancellations to avoid a $50 late fee.

Your health information is strictly confidential and protected under federal law.

Refill requests take 48-72 business hours to process through our pharmacy portal.

Our patient coordinators are available Mon-Fri, 8am-8pm.
